Quality Transfer of Care - LHPCP
Quality Transfer of Care is a project undertaken by the Lower Hume Primary Care Partnership. Members are from the various Health Care agencies including (but not limited to) Hospitals, Community Health Services and Division of General Practice
Members of the Connected Community are encouraged to share resources, plans, and collaborate to develop new resources to improve the transfer of care from Hospital to Community Services and to Home. Connected Community members can send messages, upload and download documents or ask questions of each other.
This Connected Community started in April 2010 and is an initiative of the SC/ICDM Steering Committee
