The my future my choice Planners' forum aims to support Planners in their work which involves developing individualised plans with younger people living in residential aged care. This is a forum for posting questions about the my future my choice planning tool or process. The Summer Foundation will make information and resources relevant to the planning process available on this site. However, we also want this forum to be collaborative and we encourage all members to share resources, provide links to relevant resources or services and tell us about great ideas for making a difference to the lives of younger people living in residential aged care.
The Department of Human Services In Victoria, Australia, have appointed 8 organisations to provide Planners to implement the my future my choice planning process. Between February and March 2007, approximately 30 Planners will go out and gather information about the care and support needs and preferences of 225 people under 50 years living in residential aged care.
