HACC Active Service Model Group
The Victorian Department of Human Services is seeking to move the HACC Program from a dependency model of service delivery, where tasks are largely done for clients, to include capacity building and restorative care to improve people's functional abilities and quality of life, and to delay or reduce their need for standard HACC services. A number of agencies are particpating in pilots to test possible interventions, identify clients for whom an active service approach is indicated, identify systemic implementation issues and conduct cost benefit analyses. This will contribute to formulating a policy proposal for the Minister's consideration in early 2007.
This group is for Active Service Model projects members to share ideas, documents, problems and successes.
