ConnectED
A project to improve the health and wellbeing of people with mental health and complex psycho-social needs who may be socially isolated and/or homeless and frequently use hospital services.
Auspiced by the City of Port Phillip and funded under the HARP Program of the Department of Human Services, the project commenced in January 2004.
